CHP - and the LIFTCos it develops - help professionals to integrate health and local authority services so they can deliver from modern, value-for-money, single-site facilities. CHP's work is a strong support for Practice Based Commissioning.

CHP is a hub for information and advice to professionals on public-private partnerships. Its 'LIFT Planning Toolkit for Primary and Social Care' is designed to help health professionals to understand the LIFT programme better.
