Community Health Partnerships helps to provide health and social care settings that increase staff satisfaction, raise productivity and improve employee retention. In the past, health and social care professionals, particularly in primary care, have often been unable to use all their skills fully because of inadequate facilities. CHP helps them to deploy all their expertise by ensuring the availability of safe, clean, appropriate and integrated facilities, maintained to a high standard over 25 years.
CHP - and the LIFTCos it develops - help professionals to integrate health and local authority services so they can deliver from modern, value-for-money, single-site facilities. CHP's work is a strong support for Practice Based Commissioning.
CHP is a hub for information and advice to professionals on public-private partnerships. Its 'LIFT Planning Toolkit for Primary and Social Care' is designed to help health professionals to understand the LIFT programme better.
