The new Nelson Health Centre in Merton opens its doors to patients on 1 April, greatly improving accessibility to health care for residents of Merton in south west London.
Through its integration of primary care, acute sector and community services this innovative £14m facility, brings care closer to home and is set to help to reduce unnecessary trips to hospital through its integration of primary and acute services.
At the heart of the primary care service delivery, two local GP practices - The Cannon Hill Lane Medical Practice and The Church Lane Practice – are forming a new single organisation that unites the best both surgeries have to offer and ensures excellent standards of care to all their patients.
On 31 March, Community Health Partnerships (CHP) will be joining NHS Merton Clinical Commissioning Group Chair, Dr Howard Freeman and other partners at a small launch event to celebrate the facilities going live on 1 April.
The planning, development and construction of Nelson Health Centre was led by Private Public Partnership (LIFT Company) South London Health Partnerships Ltd (SLHP), in partnership with Community Health Partnerships (CHP) and Merton CCG.
