Community Health Partnerships is happy to announce the financial close of the 47th Local Improvement Finance Trust Company (LIFTCo). Assemble Community Partnership is a newly formed LIFTCo that will begin work in the next few weeks to transform local health centres and GP Surgeries throughout the South East Midlands area.
On 1st July 2008, Community Health Partnerships, Bedfordshire PCT, East & North Hertfordshire PCT, Milton Keynes PCT, Guildhouse (a private development company) and local public sector authorities covered by the three participant PCT areas, signed a 20-year partnering agreement. Assemble Community Partnership is the name of the newly formed local partnership company.
This is the largest LIFT Partnership to be signed with 18 signatures to the agreement. The LIFT Company will work in close cooperation with Commissioners and Providers to offer partners - from public, independent contractor and voluntary sectors - the opportunity to develop a range of service solutions.
These service solutions, which include new build, remodelled and refurbished health facilities, will play a fundamental role in enhancing the quality of health and social care in the South East Midlands.
Assemble Community Partnership are currently looking at delivering a number of important proposals which include:
Bedford Health Village
Phase 1 will provide an Enhanced Services Centre to deliver a range of high quality health and social care facilities in a new, modern and highly efficient building. Future phases include the possibility of new GP facilities and the refurbishment of Shires House (a grade II listed building).
Wolverton Health Centre
The existing health centre in Wolverton is being rebuilt as part of a larger development including the leisure centre and local school. This will bring together a number of services on one site which will complement the GP services planning to re-locate to the centre, together with other health and related services.
Local General Hospital in Welwyn Garden City
Assemble Community Partnership will play a significant role in developing one of the flagship developments as part of the implementation of the strategic health care changes across Hertfordshire. Following the successful consultation 'Delivering Quality Healthcare for Hertfordshire', this large new facility will bring together primary and secondary services into a health and social care campus.
Stevenage Centres for Primary Care
Providing an integral part of E&N Herts PCT's strategic plan to deliver care closer to home, two centres for primary care are currently being developed for delivery of services to their communities in 2011. These will bring together GP's and community healthcare services into new buildings to provide high quality healthcare.
